United Healthcare is our health plan administrator, should you have any questions regarding the health plan, you may contact United Healthcare via their toll free customer service number (800-736-1264) or you may log onto their website www.myuhc.com. Members may call and chat with a registered nurse concerning their medical issues at 1.800.563.0689, which is available 24/7. This service is also accessible anytime by logging on to www.myuhc.com and clicking the link “CHAT online now” in the “Ask a Nurse” section.

The Employee Benefit Connections (EBC), provides in-house administrative functions regarding benefit enrollments, terminations, transfers and billing issues. The associates also provide the pastors and local administrators with basic benefit enrollment information packets. CVS/Caremark is our prescription drug administrator. Their customer care team can be contacted by calling (800-565-7091) or you may log on to their website www.caremark.com. You can access information regarding your coverage, order prescription refills, view your prescription history and print forms.
