United Healthcare is our health plan administrator, should you have any questions regarding the health plan, you may contact United Healthcare via their toll free customer service number (800-736-1264) or you may log onto their website www.myuhc.com. Amongst the many tools at your disposal, you can review benefit coverage, view your current and previous claims, find doctors and facilities, print claim forms, and research health conditions and treatments.The following summaries reflect the medical benefits effective January 1st, 2014:

Another important feature offered through the United Healthcare medical plan, is the availability of the Nurseline Service. Members may call and chat with a registered nurse concerning their medical issues at 1.800.563.0689, which is available 24/7. This service is also accessible anytime by logging on to www.myuhc.com and clicking the link “CHAT online now” in the “Ask a Nurse” section.

The following Uniform Glossary of Health Coverage & Medical Terms summary is a resource to help plan participants understand the most common terms used in health insurance documents, such as “deductible” and “copay”:

CVS/Caremark is our prescription drug administrator. The customer care team can be contacted by calling (800-565-7091) or members may log on to the website www.caremark.com.  Members can also access information regarding their benefit coverage, order replacement prescription cards, view their prescription history and print forms.

The Summary Plan Description outlines information regarding the current medical benefit plan.  We have posted below a Summary Plan Description for the Bargaining Lay Employees.  The SPDs for Religious & Non Bargaining Lay Employees and Clergy & Seminarians will be posted shortly as they are currently being reviewed:

The Employee Benefit Connections (EBC)provides in-house administrative functions regarding benefit enrollments, terminations, transfers and billing issues.  The associates also provide the pastors and local administrators with basic benefit enrollment information packets.  For reference, please refer to the posted forms below, which are the most common and requested:
