United Healthcare is our health plan administrator, should you have any questions regarding the health plan, you may contact United Healthcare via their toll free customer service number (800-736-1264) or you may log onto their website www.myuhc.com. Amongst the many tools at your disposal, you can review benefit coverage, view your current and previous claims, find doctors and facilities, print claim forms, and research health conditions and treatments.The following summaries reflect the medical benefits which became effective January 1st, 2013:

CVS/Caremark is our prescription drug administrator. Their customer care team can be contacted by calling (800-565-7091) or you may log on to their website www.caremark.com. You can access information regarding your coverage, order prescription refills, view your prescription history and print forms.
